| Scheduling an
Appointment |
| When you call for an appointment, our schedulers will ask for
information regarding the purpose of your visit. This will assist us in
scheduling the best time and length of your appointment.
Your appointment can be scheduled either by your doctor's office, or by
yourself. In either case, you will need a referral form from your doctor
and you may need pre-authorization from your insurance company or HMO.

| Preparing for your Appointment |
When you schedule your appointment, you will receive information about
what to bring and what, if any, preparation will be necessary for your
study. Please be sure to bring the following:
- Social Security Number
- Insurance information - If you have an insurance card, bring
it. In order to bill your insurance carrier(s) for services rendered, we
will need the name of the carrier (company), their complete mailing
address, your policy's group number, and your personal identification
number. If your insurance carrier requires a referral form, please bring
it with you to your first appointment. If you are covered by Medicare or
Medicaid, please bring the appropriate card as well.
- X-rays - If you have any relevant X-rays performed at any
office or hospital other than Western Kentucky Diagnostic Imaging,
please bring them to your appointment so our radiologists can compare
them with your new study.
- Form of Payment - You may be asked to pay for services not
covered by insurance. We accept cash, check, or major credit card (Visa,
MasterCard, Discover, or American Express).
